In MySQL, the statement to build an index is as follows: [UNIQUE|FULLTEXT|SPATIAL] CREATE index name INDEX APPLICATION OF [BTREE | HASH | RTREE] (column name [(length)] ON table name [ASC | DESC] UNIQUE specifies that MySQL will create a constraint requiring that all values in the index be distinct.
